Procedures

  1. The applicant shall submit the form and the enclosed documents to the Reception Desk officer who will check and verify the documents for completeness.
  2. The applicant will receive a payment voucher showing the due amount and obtain a turn number/queue ticket.
  3. Go to the application intake counter to validate the information against the application and duly register it on the computer system.
  4. Settle the legally required fees at the cashier’s counter.
  5. The printing officer will print the plastic cards and then gives the new health card to the applicant

Required Documents

  • The duly filled out form of issuance/renewal.
  • A cover letter from the Prime Ministry, the Jordanian Senate, and the House of Representatives.
  • A liabilities certificate to deduct (50) JODs for the health insurance allowance.
  • A copy of the Family Book or the Civil Status ID Card.
  • One (1) personal photograph for every beneficiary and subscriber.
